Our team wants to help keep your student safe and healthy for sports. When you come in for a sports physical, a HealthNet provider will check your student’s health and make sure they are ready to play. We hope your visit is quick, comfortable, and helpful, so you feel good about your student starting their sports season.
What can I expect from a sports physical?
Our team and providers will check your students health and make sure they are ready to play! We will:
- Review your student’s medical history and may discuss vaccines.
- Check your student’s height, weight, blood pressure, and other important vitals.
- Check to see how your student moves, stands, and how their heart, lungs, and joints are working.
- Complete a simple vision screening, but you may also want to schedule a more comprehensive eye exam at HealthNet’s Eye Care Center.
- Talk about healthy choices and good habits.
Sports physicals are not the same as a full check-up. If you want a full exam, please let us know and we can help set one up.
Students under 18 must have a parent or guardian with them.
Can a sports physical be completed as part of an annual wellness exam?
Yes! It is important that ever student has a comprehensive health exam every year. Wellness checks include things that are part of a sports physical.
What should I bring?
Please bring a copy of the required forms provided by the school, sports team, or camp. Indiana students in grades 9-12 playing under IHSAA guidance can access the IHSAA Pre-Participation Evaluation Form (PPR) here.
