Dear Patients,

Thank you for the trust you have given me over the years as your Behavioral Health Nurse Practitioner. Working together to help you reach your goals has been an honor.

I’m writing to let you know that I will be leaving HealthNet on November 17, 2025. If you would like an appointment, please call us at (317) 957-2070. We will schedule an appointment for you as quickly as possible, either with me before I leave or with another behavioral health provider of your choice.
HealthNet has several health centers nearby. Visit our website at indyhealthnet.org/locations to learn more. Transportation assistance is available and can be arranged for you.

If you already have an appointment scheduled before November 17, I look forward to seeing you then. If you are scheduled to see me after November 17, a health center team member will contact you soon to reschedule your appointment with another provider.
