Dear Patients,

Thank you for the trust you have given me over the years as your doctor. Taking care of you has been an honor.

I’m writing to let you know that I will be leaving HealthNet Barrington Health & Dental Center on June 20, 2025. I am pleased to share with you that my colleagues will be taking on my responsibilities when I leave:

You are welcome to continue your care with any of these providers at HealthNet Barrington Health & Dental Center, at another HealthNet location, or with any doctor you choose. If you need help with transportation, we can assist you.

I will work closely with these providers during this transition. Your new provider will have all the information I have charted regarding your care. We will remind you again about my transition when your next appointment is due. Please call and talk with us at (317) 957-2100 about any of your concerns.
