An Important Message from Dr. Alex Geiger

Dear Patients,

Thank you for trusting me as your provider. It has been an honor to care for you.

I want to let you know that my last day at HealthNet Southwest Health & Dental Center will be June 6, 2025. While I will miss you, I am happy to share that we are hiring a new provider to take my place in the coming months.

Until then, other HealthNet providers at Southwest and our other health centers are ready to care for you. Please see the attached provider list and health center map.

• If you have an appointment with me before June 6th, I look forward to seeing you.
• If your appointment is after June 6th, we will call you to reschedule with another provider or move up your appointment so that I can see you before I leave.

You can continue your care with another provider at Southwest, at another HealthNet location, or any doctor you choose, depending on appointment availability that best meets your needs. If you need help with transportation, we can assist you.

I will work with my team to make this change easy for you. Your new provider will have all the information I have charted regarding your care. We will also remind you about this change before your next appointment.
