Step 1: Call 317-957-2070 to talk to a Patient Access Specialist. If you have insurance, please have your insurance card with you when you call. 

On this call, our Patient Access Specialists may...
 Ask you about your current health needs and schedule you with one of our skilled providers.
 Talk to you about a sliding fee scale discount to help lower the cost of your care.
 Sign you up for voice or text messages. This helps us share important information with you.
 Explain how to transfer your medical records from your old doctor to HealthNet. 
 Answer any questions about our New Patient process and services.

Step 2: Help us get to know you.

Help your new provider get to know you by completing this Medical Records Transfer Form. Please give this form to your old doctor before your first visit at HealthNet. This will allow your old doctor to share your medical records with us. 

If your old doctor sends you your medical records, please bring them with you to your visit. 

Step 3: Prepare for your visit. 

You may be able to get a discount to help lower the cost of your care through our sliding fee scale. Click here for the Proof of Information Checklist to view what items to bring with you to find out if you can get a discount. 

Also, please bring all current medicine to your first doctor's visit.
