Dear Patient,

Thank you for the trust you have given me over the years as your provider. Taking care of you has been an honor.

I’m writing to let you know that I will be leaving HealthNet Bloomington Health Center on March 1, 2024, and moving to the HealthNet Mobile Health Center. While I will miss you all, I look forward to providing primary care services at various locations around Monroe and Morgan counties.

I am pleased to share with you that my colleagues will be taking on my responsibilities when I depart:

You are welcome to continue your care with any of these providers at HealthNet Bloomington Health Center, or with any provider of your choice. In addition, you are welcome to see me at the HealthNet Mobile Health Center, which regularly stops at various locations in Bloomington, Ellettsville and Martinsville.
