HealthNet’s Connected Care Program can help you manage your health and spend more time doing the things you enjoy, in good health.


If you have Medicare and 2 or more chronic conditions like arthritis, diabetes, or depression, you are eligible for HealthNet’s Connected Care Program. Benefits of this program include: 

  • Help managing your medicine and refills
  • Management of your care and visits with providers, hospitals, labs, x-rays, and more
  • More support between your visits. You will have direct access to HealthNet’s Connected Care nurse to help manage follow-ups or to answer your questions.   
  • Saving money! Better care management can help you stay healthy, avoid trips to the ER and prevent falls.
  • Building a relationship with your Connected Care team who knows you and your health conditions.

Do I have to make an appointment or come in to see my provider to get Connected Care services?

No! Connected Care services can be done over phone, at your health center, or in your home. Talk with your Connected Care nurse about what works best for you! 

Who can provide Connected Care services?

Your Connected Care nurse can provide you with other resources to help improve your health. HealthNet offers other services like diabetes education, medication management, health coaching, and nutrition counseling.

How often will I talk to my Connected Care nurse?

When you sign up for Connected Care you will have regular access to your nurse. You will get regular phone calls from your Connected Care nurse to talk about your current health and answer any questions you may have.
